Places are limited to 16 teams per tournament only;
Each team must consist of 5 members.
PRIZES:
Champion: RM 2000 + 5x The Orange Box + 5x Medals
1st runners up: RM1000 + 5x The Orange Box + 5x Medals
2nd runners up: RM500 + 5x The Orange Box + 5x Medals
REGISTRATION FEE:
Registration fee for both tournaments is RM60 per team.

Captain's name,
(2)
Handphone number,
(3) Email address, and the
(4) Names of the other 4 members
